| Office Overhead Expense Insurance Summary |
Office Overhead Insurance is intended to provide a monthly benefit to help you meet the ordinary and fixed office expenses that continue in your absence in the event you become totally disabled as the result of a covered sickness or accident. Maintaining these expenses in your absence is vitally important and this benefit may prevent you having to decrease staff, resort to savings or investments, or rely upon disability benefits to pay the overhead expenses. The normal and regular business expenses are covered under the plan.
In addition, the plan includes a Survivor Benefit which provides a one sum payment to your spouse, if living, and otherwise to your children. The benefit is equal to the lesser of (1) 3 times your Maximum Monthly Benefit and (2) 50% of your Maximum Monthly Benefit times the number of months remaining in the unexpired portion of the Maximum Benefit Duration at the time of your death.
The plan will pay 100% of your actual covered expenses up to the plan benefit maximum of $5,000 per month. Coverage is available to all members of the State Bar of Texas who are less than age 70.

HOW TO ENROLL
You may request more complete information by using our online request form or download an enrollment form (pdf format).
All applicants will need to complete a Prudential Application Form, including the reverse side of the Health Statement (Part C) if requesting coverage on dependents. Approval process takes approximately 30 days, depending upon additional information requested by Prudential. Return the completed form to:

After you enroll, you will be issued a Group Insurance Certificate describing your coverage in greater detail. The complete terms of the coverage will be governed by the group insurance contract issued to the State Bar of Texas Insurance Trust by the carrier.
